For those transferring from a Fidelity account to Christ Church’s Fidelity account:

• If the value of your donation is $19,000 (individual) or $28,000 (couple) or less, you can call Fidelity and verbally ask to transfer the amount over the phone.
• If the value is greater than noted above, please complete this form.

For those transferring from a non-Fidelity account to Christ Church’s Fidelity account:

• The attached form includes the securities settlement instructions that can be used to execute your trade with your broker. Please provide a copy of the trade details (either this completed form or one provided by your broker to Lana Ferren at lferren@ChristChurch.
